Enrollment Information

Prospective parents please complete this Enrollment form to register your child for our school. Registration occurs every Monday by appointment only. Mrs. Cunningham will contact you to set your virtual appointment.

Due to Covid-19, the steps for registration/enrollment and transfers have changed to the ensure the safety of families and staff. If you are planning a new enrollment, please review the document requirements below. Afterwards, click the Bennett Pre-Registration link to enter student information. A school clerk will call you for an appointment to complete your registration/enrollment process within 48-72 hours.

Student Enrollment & Registration Requirements (Neighborhood)


A parent/guardian must enroll a student. The parent/guardian must present the following:

  • Original birth certificate,

  • Proof of guardianship (You will be asked to present documentation to verify guardianship)

  • 2 proofs of address ( Current utility bill, Illinois driver's license or Illinois ID, Lease, deed, MediPlan/Medicaid card, court documents, Illinois Department of Public Aid card, Stamped United States Post Office change of address form, Illinois state aid check/social security check)

  • Medical Forms – CPS HEALTH REQUIREMENTS include the following

    • Physical

    • Shot Records

    • Dental, Hearing and Vision Screening.

    • New physicals are required for Pre-K, Kindergarten, 1st grade, 6th grade, & 9th grade (Required if new student to CPS or transferring into Illinois)


All students attending Bennett Elementary School should live within the school’s attendance boundaries. Any child whose parent enrolls them under misrepresentation of information will be investigated and transferred immediately.


Pre-K Enrollment at Frank I. Bennett Elementary

  • The application for Pre-K at Bennett elementary is now open. Go to Chicago Early Learning to fill out and complete the application.

  • Once the application is completed you will be called into Bennett for an appointment to complete the application process.

  • Open to students in the attendance boundary
